Coordinated Veterans Care (CVC) Program
The Coordinated Veterans Care (CVC) Program is a key initiative designed to provide structured, team-based care for eligible veterans.
It places you at the centre of your healthcare by bringing together the three most important members of your care team:
• You (the patient)
• Your General Practitioner
• A dedicated Nurse Coordinator Working collaboratively, this team develops a personalised care plan focused on your individual health needs, goals, and long-term wellbeing

Gold Card holders
Eligible White Card holders with a diagnosed mental health condition
