The Coordinated Veteran Care (CVC) Program is a key initiative designed to provide team-based care for eligible veterans. It empowers you by bringing together the three most important members of your healthcare journey:

· You (the patient)

· Your GP

· A dedicated Nurse Coordinator

Working in partnership, this team creates a tailored approach to achieve your patient-centred healthcare goals and support your optimal health and wellbeing.

Eligibility

The CVC program is available to:

· Gold Card holders

· Eligible White Card holders with a mental health diagnosis
